Volunteer Waiver & Liability Release

Safety is our top priority. To participate in the Adopt a Park or Trail program, every volunteer must have a signed waiver on file before their first cleanup event.

For Group Leaders

If you are the main contact for your group, you are responsible for ensuring every participant has completed this form before your scheduled cleanup day.

Waiver Requirements

  • Individual Forms: Each person volunteering must submit their own information.
  • Minors: Volunteers under the age of 18 must have a parent or legal guardian sign their form.
  • Annual Updates: Waivers should be renewed once per year to ensure contact information is current.
