Please provide the following documents to complete your application. You can upload digital copies or bring physical copies to our office.
1. Proof of West Des Moines Residency
Provide a copy of one of the following showing your name and current address:
- Utility bill (Water, Electric, Gas, Cable, or Cell Phone)
- Bank statement
- Current lease agreement
- Paystub or proof of eligibility from another assistance program
2. Proof of Income
Provide documentation for all adults (18+) in the household:
- Tax Returns: Your most recent federal tax return. (If you do not file taxes, please ask us for a Tax Waiver Certification form).
- Employment: Your last 30 days of paystubs.
- Social Security: A copy of your current award letter. If you need a copy, call 1-800-772-1213.
- Other Income: Documentation for child support, unemployment, pensions, or rental income.
3. Proof of Assets
Provide documentation for all adults (18+) in the household:
- Bank Statements: The first page only of your most recent statement for all checking and savings accounts.
- Investments: Your most recent statement for any stocks, bonds, IRAs, 401(k)s, or IPERS accounts.
