Accommodation Request Form

The City of West Des Moines wants to make sure everyone can take part in our programs and services. If you need a specific change or accommodation—like a sign language interpreter, materials in large print, or a physical adjustment—please let us know by filling out this form.

Please submit your request at least 72 hours (3 days) before the event. This gives us enough time to make the necessary arrangements.
